Job Description

The South East Grey Community Health Centre is a non-profit organization committed to providing primary care services and health programs to the residents of the Municipalities of Chatsworth, Grey Highlands, Southgate and West Grey. We are committed to building a healthier South East Grey community through: health promotion, illness prevention, and comprehensive, client-centered primary health care.

 

SEGCHC is currently seeking a Full-time Social Worker. Reporting to the Director of Allied Health, the Social Worker will work as part of a dynamic, inter-professional team, providing patient advocacy, counselling/therapy and other therapeutic functions. The Social Worker will participate in the development, implementation, monitoring and evaluation of programs and services for individuals, families and communities.

Education and Qualifications:

  • Masters of Social Work degree from a recognized University
  • Current membership and in good standing with the OCSWSSW
  • Knowledge and proficiency in current, evidenced-based methods and practices of social work in primary care settings, with an emphasis on chronic disease management, health promotion and risk reduction
  • Addictions and substance use counseling considered an asset
  • Demonstrated experience of case management, system navigation and advocacy
  • Working knowledge of relevant health care legislation and community mental health resources
  • Experience with creating, implementing, facilitating and evaluating groups
  • Experience in an inter-professional setting will be considered an asset
  • Understanding of social determinants of health and their impact on health and well-being
  • Efficient computer skills, working knowledge of electronic medical record and documentation
