Job Description

The South East Grey Community Health Centre (SEGCHC) is a primary care facility located in Markdale, ON. SEGCHC is dedicated to meeting the broader needs of the communities in which we serve through the development and implementation of primary care, health promotion and community development best practices.

 

SEGCHC is currently seeking a full-time, temporary 18-month contract Registered Dietitian (37.5 hours per week). The Registered Dietitian will provide a balance of nutrition counselling and community programming.

 

Key responsibilities include:

 

 

  1. Performs nutritional assessments, development of client-focused nutritional plans, health education/counselling and other clinical activities in accordance with the College of Dietitians of Ontario standards of
  2. Perform community-based nutrition needs assessments and identify barriers to acquiring adequate food security and facilitate and support community initiatives to improve food
  3. Contribute to chart documentation, participation in chart reviews and case conferences, and make referrals to other clinical organizations/individuals as
  4. Participate in the development, implementation, monitoring and evaluation of treatment education, counselling and health promotion programs and services for individuals, families and the

Education and experience required:

 

  1. Registration in good standing with the College of Dietitians of Ontario.
  2. 3-5 years of clinical practice experience preferred.
  3. Strong knowledge of nutritional issues affecting clients.
  4. Excellent communication and presentation skills with a demonstrated ability to undertake nutritional counselling (individual and group)
  5. Demonstrated ability to work independently and within an inter-professional team.
  6. Contribute to chart documentation, participate in chart reviews and case conferences, and make referrals to other clinical organizations/services as required.
  7. Identify barriers to food security; facilitate/support community initiatives to improve food security.
  8. Strong foundation in health promotion, health equity and the social determinants of health.
  9. Excellent interpersonal, time management and organizational skills.
  10. Strong skills in critical thinking and problem-solving.
  11. Proficiency in the use of computers and various software applications including electronic medical records and Microsoft Office.
  12. Possess a valid driver’s license with access to a reliable vehicle.
  13. Current Police Check with Vulnerable Sectors Screen.
  14. Proof of current COVID 19 vaccination (this includes at least one additional booster).
