Support Worker- Supported Group Living
All aspects of personal care including but not limited to
- Washing/bathing
- Mouth care
- Hair care
- Menstrual care
- Preventative skin care
- Changing dressings
- Routine hand/foot care
- Transferring/positioning/turning
- Transfer to and from a vehicle
- Dressing/undressing
- Assistance with eating, oral and tube
- Toileting (including empty/change leg bag, catheterisation), bowel routines
- Assisting the person to take medications
- Range of motion
- HEALTH AND WELLENESS promotes the health and wellness of all clients
-Respiratory care where the needs of the person are stable.
-Glucose monitoring
-Assisting with communications.
-Assist clients with taking medications accurately and in accordance with agency policy and procedures
-Observes and implements appropriate actions to promote healthy living and prevent illness and accidents
-uses appropriate first aid/ safety procedures when responding to emergencies
-assists in scheduling, keeping and following through on all health appointments
-assists with identifying securing and using needed adaptive equipment and therapies (e.g. physical, occupational, speech, respiratory, psychological)
-Transport and accompany individuals to medical, dental and related appointments,

-Physical Development and Health
e.g. Hydrotherapy, physio etc
-Sensory-motor Development
e.g., development programming etc.
-Behavioural Management
2. HOUSEHOLD MANAGEMENT:
Assist with household management (i.e. meal preparation, laundry, light housekeeping, cleaning, decorating, general tidying of clients personal living space(s)
Coordinates necessary shopping activities
Ensures maintenance and safety of building and equipment
Other services that the Ministry may prescribe as homemaking services
3. Transportation / Vehicle maintenance
Transports clients to appointments, community events
Assists individuals in arranging transportation for appointments and events
Maintains legal full G class drivers license and acceptable driving record as per agency insurance carrier, informs agency immediately if any change
4. Community and service networking
Staff are knowledgeable about the formal and informal support available in his or her community and is skilled in assisting the individual to identify and gain access to such support
Staff support the individual in development of friendship and other relationships
5. Crisis intervention and health and safety
Know and follow health and safety policies and procedures
Always demonstrate safe work practices
Identify, report and follow up on items that need to be repaired
Reports all unsafe conditions, practices equipment.
Reports on all accidents and completes incident reports
Knowledgeable about crisis prevention, intervention and resolution techniques
May be required to stay at or return to work during public health incidents and emergencies to perform duties specific to this classification or to perform other duties as requested in an assigned response position. This may require working a non-traditional work schedule or working outside normal assigned duties during the incident and / or emergency.

JOB SPECIFICIATIONS
QUALIFICATIONS:
Education, Training and Experience:
- PSW, DSW or HCA
- Ability to perform shift work requirements in a 24 hour a day, “7 day a week” operation,
- Physically and mentally capable of performing assigned duties,
- Experience working with people with physical and developmental disabilities preferred
- Formal or informal education or training which ensures the ability to read and write at a level necessary for successful job performance.
- Must have CPR and First Aid,
- Must have or be willing to obtain non-violent crisis intervention training.
- Acceptable criminal reference check.
- Valid “G” driver’s license with acceptable driving record (as per agency insurance requirements) and reliable vehicle
- Proven work performance, effective communications skills (including written and verbal, interpersonal and organizational skills and ability to work independently and as part of a team.
ABILITIES
