OUTREACH SUPPORT WORKER

Outreach Manager

JOB SUMMARY:

 

Reports to the manager of Outreach, to provide hands-on attendant services to clients served by Participation Lodge Grey Bruce. This position has a primary responsibility for the day-to-day support of individuals receiving services including personal support, independence training, and personal outcomes in their homes and in the bathing program.

 

Key Responsibilities

  1. All aspects of personal care including but not limited to:
    1. Washing/bathing
    2. Mouth care
    3. Hair care
    4. Menstrual care
    5. Preventative skin care
    6. Changing dressings
    7. Routine hand/foot care
    8. Transferring/positioning/turning
    9. Dressing/undressing
    10. Assistance with eating, oral and tube
    11. Toileting (including empty/change leg bag, catheterization), bowel routines
    12. Assisting the person to take medications
    13. Range of motion:Assists clients with taking medications accurately and in accordance with agency policy and procedures
    14. Observes and implements appropriate actions to promote heathy living and prevent illness and accidents
    15. Uses appropriate first aid/ safety procedures when responding to emergencies
    16. Assists with identifying securing and using needed adaptive equipment
    17. Behavioral management: Assist individuals with household management (i.e meal preparation, laundry, light housekeeping, decorating, general tidying of individuals personal living space(s)
    18. Ensures maintenance and safety of program equipment
    19. Enhance the ability of the individual to lead a self-determining life.
    20. Understands and communicates in the individual needs, wants and choices to their family, coworkers the organization and the system
    21. Provides encouragement to individual regarding the realization of their hopes, dreams, and choices.
    22. Response to complaints voiced by clients in a timely respective manner

 

 

Qualifications

  • PSW certificate or DSW certificate
  • Have and maintain a “G” driver’s license and acceptable driving record (as per agency insurance requirements)
  • Valid CPR First Aid
  • Current Non-violent crisis intervention training
  • Valid Criminal reference check for vulnerable sector
  • Willingness to complete Medication administration training

 

Competencies

  • Strong written, verbal and listening skills
  • Experience working with persons with physical and developmental disabilities preferred
  • Excellent judgment in dealing with sensitive and confidential information in a diplomatic, professional, and tactful manner
  • Maintains standards of confidentiality and ethical practice according to PLGB Code of Conduct
  • Self-directed with the ability to work independently and as part of a team
  • Ability to work professionally, effectively, and co-operatively with all individuals to build relationships
  • Demonstrated computer skills in Microsoft Office
  • Strong organizational skills with ability to respond to multiple tasks and competing priorities

 

Working Conditions

  • Ability to lift and carry equipment and supplies weighing up to 55 pounds.
  • Ability to walk, kneel, climb, stand, stoop, crouch, and reach when duties.
  • Any employee may be required to stay at or return to work during public health incidents and/or emergencies to perform duties specific to this classification or to perform other duties as requested in an assigned response position. This may require working a non-traditional work schedule or working outside normal assigned duties during the incident and/or emergency.
  • Ethical use of sick time and LOA’s, punctual start and end shift times
  • Represents agency in a positive manner to families, support network members and the community
