Job Description
The Community Support Services Coordinator is responsible for the development and implementation of the Home and Community Care Support Services Day Program. This role is to ensure that all Clients and elderly Community Members have access to weekly and special day program activities. As part of this role, you would find yourself conducting home visits to inform and encourage participation in programs and events, providing social stimulation with conversation, playing games and exercising in or outside of their home.
Qualifications:
- High School diploma or equivalent
- Two (2) years’ work experience in a healthcare setting
- Two (2) years’ experience with First Nations
- Knowledge of healthcare issues within First Nations
- Competency with computers and Microsoft Office software
- Understanding and/or experience with programming
- Strong communication and customer service skills
- Financial literacy for budgets, reports, statistics, etc
- Valid Ontario Class F Driver’s Licence or ability to obtain within three (3) months
- Valid CPR First Aid certification or proof of being registered to obtain within three (3) months
Equivalencies that include a combination of education and experience may be considered.
